Wisconsin Form UC-101A in QuickBooks: What Gets Prefilled and What You Fix

QuickBooks prefills most of Wisconsin's quarterly unemployment wage report, but formatting gaps and missing state data can leave fields blank.

QuickBooks Desktop handles Wisconsin’s quarterly unemployment contribution and wage report — Form UC-101A — by prefilling most fields automatically, but employers running the form for the first time routinely run into gaps where data is missing or formatted incorrectly. The accepted guidance walks through what the software fills in, what it does not, and where the numbers come from.

Who Uses This Form

Form UC-101A is the quarterly contribution and wage report required by the Wisconsin Department of Workforce Development from covered employers with fewer than 10 employees. Larger employers — those with 10 or more employees — file Form UCT-101 along with Form UC-7823 instead. QuickBooks generates the UC-101A from the payroll data already in your company file, and in most cases, if your company, payroll, and employee records are complete and up to date, the form requires little or no manual entry.

What QuickBooks Fills Automatically

The software populates employer identifying information — federal EIN, phone number, legal name, and address — directly from your company setup. It also enters each month of the quarter along with the number of employees who worked on the 12th of each month. If there was no employment during a payroll period in a given month, the program enters zero.

Total covered wages flow from the wage detail later on the form itself — specifically line 25 — so that figure is calculated rather than typed. The software also computes the wage exclusion on line 9 by subtracting line 10 from line 8.

Fields You May Need to Enter Manually

Several fields cannot be derived from your QuickBooks data and require manual input:

Internet Access Number. This is a six-digit number assigned by the DWD for online filing. It appears on the preprinted UC-101A the state mailed to you, and QuickBooks has no way to generate it on its own.

Interest due. If you are filing after the deadline, interest accrues at one percent per month — or fraction of a month — on the tax shown on line 12. You must calculate and enter this amount yourself.

Late filing penalty. A flat $25 penalty applies to any report filed after its due date.

EFT payment. If you paid your quarterly taxes by electronic funds transfer, enter that payment amount on line 15.

Credit available. If the DWD notified you of any available credit, enter both the date and the amount on line 16.

Health insurance checkboxes. Mark the appropriate box to indicate whether you offer health insurance coverage.

The SSN Formatting Problem

One of the more common stumbling blocks involves Social Security number formatting. Wisconsin’s scanning equipment requires SSNs on the form to appear without dashes. QuickBooks stores and displays employee SSNs with dashes in the normal employee setup screens, so users who manually adjust or verify the form need to be aware that the printed output should strip those dashes for the state’s processing system. The form generation in QuickBooks is designed to handle this formatting automatically — if dashes are appearing on the printed form, that is a sign something in the employee record or form setup needs attention.

Correcting Employee Information

If employee details on the form are wrong — a misspelled name, an incorrect SSN, or an outdated address — the fix is not on the form itself. You need to go back into QuickBooks, correct the employee record in the Employee Center, and then regenerate the form. The UC-101A pulls directly from those records, so any change must happen at the source.

Employer Information Changes

If your business has a new address, phone number, or legal name, correcting it in QuickBooks updates the form going forward, but the DWD also wants to know. Complete Form UCT-6491, Account Change Information, and return it with your quarterly report.

Where to File

If payment is due, mail the completed form with your check or money order — payable to the Division of Unemployment Insurance — to the DWD’s lockbox in Milwaukee. If no payment is due, the form goes to the DWD’s Madison office instead. The two addresses are different, so check which situation applies before mailing.
