QuickBooks Desktop includes built-in support for South Dakota’s unemployment insurance quarterly filing, but employers with larger payrolls frequently ask how the continuation sheet works and which fields they are responsible for completing. The form in question — DOL-UID-21, the Continuation Sheet for the South Dakota Employer’s Quarterly Contribution, Investment Fee, and Wage Report — comes into play when a company needs to report more than 10 employees on a single quarterly return. Here is what the community has established about generating the form and what QuickBooks does and does not fill in automatically.

When the Continuation Sheet Applies

The DOL-UID-21 continuation sheet is an extension of South Dakota’s primary quarterly wage report. Employers who have more than 10 employees to report during the quarter will see additional pages generated to accommodate the overflow of employee-level wage data. The continuation sheet captures the same per-employee detail as the main form — Social Security numbers, names, gross wages, and excess wages — but for employees beyond the initial 10 listed on page one.

A key structural detail: the totals from all continuation pages roll up into the summary amounts on the first page of the report. You do not need to manually add continuation-page figures into the page-one totals; QuickBooks handles that aggregation when the full report is generated.

Generating the Form in QuickBooks

To produce the DOL-UID-21 continuation sheet, you will work through QuickBooks Desktop’s state payroll tax form workflow. The form is accessed from the Employees menu by selecting Payroll Forms and W-2s and then Process Payroll Forms. From the list of available forms, choose the South Dakota quarterly unemployment contribution and wage report. QuickBooks will generate the complete packet — the primary page followed by as many continuation sheets as your employee count requires — based on the payroll data you have already entered for the quarter.

Before running the form, confirm that your company’s South Dakota unemployment insurance account number is entered in your QuickBooks company profile. The program pulls that account number automatically and places it on each page of the generated report. The quarter and year-end date are likewise populated based on the reporting period you select when launching the form.

What QuickBooks Prefills Automatically

  • Account Number and Quarter/Year Ending: Both are derived from your QuickBooks company setup and the reporting period you select.
  • Item 15 — Employee Social Security Number: Each employee’s SSN is pulled from individual employee records.
  • Item 16 — Employee Name: Displayed in last-name-first format, sourced from employee setup records.
  • Item 17 — Total Gross Wages Paid This Quarter: The program calculates each employee’s quarterly gross wages from paycheck data.
  • Item 18 — Excess Wages This Quarter: QuickBooks computes the portion of each employee’s gross wages that exceeds South Dakota’s current taxable wage base for the calendar year. This figure is capped so that it never exceeds the corresponding amount in Item 17 for the same employee.
  • Gross Wages This Page and Total Excess Wages This Page: These subtotals are calculated as the sum of all Item 17 and Item 18 entries, respectively, for the employees listed on that specific continuation page.

Fields That May Require Manual Entry

While QuickBooks handles the bulk of the data entry, the form window flags certain fields with an alert if the program cannot derive a value from your existing data. These flagged lines are where you will need to enter an amount — or a zero, if the line does not apply to your situation. The guidance notes that in most cases where all company, payroll, and employee information has been fully and accurately entered into QuickBooks ahead of time, no additional manual input will be necessary.

If you find that fields are not populating as expected, the most common cause is missing or incomplete data in your employee or payroll records. Verifying that each employee’s Social Security number, wage setup, and quarterly paychecks are correctly recorded will resolve most blank-field issues on the generated form.

Verifying the Numbers

For employers who want to trace where a specific figure on the DOL-UID-21 originated within QuickBooks, the form window includes a Help button. Selecting it while a specific field is active brings up context-specific guidance explaining how the program calculated that value from your underlying payroll data. This is also the recommended starting point if a number on the form does not match your own records — the Help resource can identify which paycheck entries or employee setup details are feeding into the calculation.
