QuickBooks Ohio Form JFS-66111 Employer's Contribution Report Explained

QuickBooks prefills most of Ohio's quarterly unemployment tax form, but certain fields and filing steps require manual review before submission.

QuickBooks Desktop includes built-in support for Ohio Form JFS-66111, the Employer’s Contribution Report used to report unemployment insurance tax to the Ohio Department of Job and Family Services (ODJFS). The form calculates tax on wages paid during the quarter and is designed to work alongside the employee wage listings on Form JFS-66113. QuickBooks generates the computer-produced version of the lower portion of the paper form ODJFS mails to employers, and most data fields populate automatically from existing company and payroll records.

Generating the Form

To produce the JFS-66111 inside QuickBooks Desktop, navigate to the Employees menu, select Payroll Forms, then Process Payroll Forms. Choose Quarterly Form 941/Schedule B or the state payroll forms section, depending on your version, and select the Ohio unemployment form from the list of available state forms. Set the filing period to the appropriate quarter and let QuickBooks pull the wage and tax data from your payroll records. Setup Prerequisites

Before opening the form, confirm that your company profile, payroll items, and employee wage details are all current inside QuickBooks. The contribution rate that appears on Line 12 of the form carries over from the rate field above Line 9, which means your Ohio unemployment tax rate must already be entered correctly in your payroll setup for the number to transfer. When all company, payroll, and employee data resides in QuickBooks and the tax rate is configured, the form generally requires little or no manual entry beyond review.

Fields QuickBooks Does Not Fill

After the form opens, QuickBooks will have populated most fields from your payroll data. Scan every field and complete anything left blank. The form window includes a Help button for guidance on individual fields and for troubleshooting. Hyperlinks within the form provide a breakdown of where each number originated in your company file, which is useful when reconciling the unemployment insurance totals against your payroll registers.

QuickBooks also offers options to summarize payroll data in a spreadsheet format and to save a copy of the completed form as a PDF for your records. Both are accessible through hyperlinks on the form screen.

Filing and Mailing

Once the form is complete and reviewed, print it and mail it with all copies of the Employer’s Report of Wages to:

Make remittances payable to “Ohio Dept. of Job and Family Services” or “ODJFS.” QuickBooks produces the computer-generated form, but the actual submission to the state remains a manual mailing step for employers using this workflow.

Penalties and Rate Details

ODJFS assesses a forfeiture of one-quarter of one percent (0.25%) of total remuneration paid — with a minimum of $50 and a maximum of $1,000 — if the report does not conform to instructions or arrives after the due date. Late contributions also accrue interest at 14% per year, compounded monthly, with any fraction of a month treated as a full month.

Updating Employer Information

Address changes or business-status updates require Form JFS-66111A, which ODJFS provides directly to employers. QuickBooks does not handle this status-change form; it must be submitted separately to the state agency.
