North Carolina employers using QuickBooks to prepare Form NC-5 — the state withholding return — have run into a recurring snag: the form refuses to calculate, and in some cases prints with a warning that there is insufficient information to complete the scanline. The root cause, according to the accepted community guidance, is almost always a missing or incorrect North Carolina withholding account number stored in the company’s QuickBooks setup.

What Form NC-5 Covers

Form NC-5 is the return North Carolina employers use to report and pay state income tax withheld from employee wages. QuickBooks generates the form using payroll data already in the company file, prefilling most fields automatically. Filing frequency depends on withholding volume: employers who withhold an average of at least $250 but less than $2,000 per month file monthly, while those below the $250 threshold file quarterly. QuickBooks determines the frequency — monthly or quarterly — based on the period dates entered for the return.

The Account Number Requirement

The most common stumbling block is the North Carolina account number. Without it, QuickBooks cannot complete the form’s calculations. Users who have not yet registered with the North Carolina Department of Revenue must obtain an account number before the form will function. Those who do not have a state account number are directed to file Form NC-5A or NC-5PA instead.

If the account number is missing or entered incorrectly, QuickBooks displays a notice on the form stating that there is insufficient information present to complete the scanline. The accepted guidance is emphatic on one point: do not mail the form without a valid scanline. Submitting a return with an incomplete scanline can trigger penalties from the state agency.

Filing Deadlines

Monthly returns and payments are due by the 15th day of the month following the withholding period, with one exception — the December return and payment are due by January 31. Quarterly returns and payments are due by the last day of the month following the close of the calendar quarter.

Penalty and Interest Lines

QuickBooks does not automatically calculate penalties or interest on Form NC-5. Line 2 is reserved for penalty amounts, which the preparer must enter manually if applicable. North Carolina imposes a 10% penalty on the required withholding amount for failure to withhold or pay on time, and a 5% per month penalty for late filing, capped at 25% of the tax due.

Line 3 covers interest on late payments, accrued at a rate set by the Secretary of Revenue. The current rate is available through the North Carolina Department of Revenue website.

What to Check in QuickBooks

The accepted solution boils down to a straightforward data-entry fix. Users experiencing calculation failures or scanline warnings should return to their QuickBooks company settings and verify that the North Carolina account number is present and correctly formatted. Once the number is added or corrected, QuickBooks can complete the form’s calculations and generate the scanline needed for processing.
