The form is built into QuickBooks and prefills most fields automatically, but several details require manual attention to avoid processing errors or rejections by the Ohio Department of Taxation.

Who Uses Form IT-501

Form IT-501 applies specifically to quarterly filers of Ohio withholding tax. Employers who remit on a monthly schedule use a different voucher — Form IT-501(M) — which is selected separately at the start of the payroll forms workflow in QuickBooks. Choosing the correct filing frequency at the outset determines which form QuickBooks generates, so users who see the wrong voucher should backtrack and verify their selection.

Electronic Payment Is Mandatory

A key point that has tripped up some users: since the 2015 tax year, Ohio requires employers to pay withholding tax electronically. Paper vouchers are no longer accepted for payment. QuickBooks supports e-payment of Form IT-501 directly through its e-file and e-pay setup, which users must configure before the filing deadline. Without that setup enabled, the payment cannot be submitted through the software.

Fields QuickBooks Fills Automatically

QuickBooks populates the majority of Form IT-501 using existing company, payroll, and employee data. In most cases, if all records are current and complete, no additional manual entry is needed. The filing status, period, and due date calculate automatically based on the form and date range selected at the beginning of the process.

Fields That Require Manual Review

Several fields either need verification or manual entry:

  • Ohio Withholding Account Number — Must be an 8-digit number in the format XX XXXXXX, beginning with the prefix 51, 52, 53, or 54. No letters are allowed. This number is entered through QuickBooks Payroll Setup or the Payroll Item List, and the correct format is essential because Ohio uses scanning equipment to process the form.

  • TIN and Scan Line — These fields are required for the state’s scanning equipment to read the form properly. They generate based on data already in QuickBooks, but users should confirm accuracy.

  • Preparer’s Social Security Number — The person responsible for remitting withheld taxes must enter their SSN. Ohio prefers the preparer’s actual Social Security number, though a federal PTIN is an acceptable alternative for third-party preparers.

Common Sticking Points

The issues users encounter most often stem from incomplete setup rather than software malfunction. An incorrect or improperly formatted withholding account number will prevent the form from processing correctly through Ohio’s scanning system. Missing e-pay enrollment means the payment cannot be submitted on time. And selecting the wrong filing frequency at the start of the workflow produces the wrong form entirely.
