QuickBooks Desktop includes a built-in New Hire Reporting Form for Iowa, but several fields on the form require manual attention or do not auto-populate from employee records — a detail that has caught employers off guard when filing with the state’s Centralized Employee Registry.
Filing Deadline and Required Information
Iowa employers are required to submit the New Hire Report to the Centralized Employee Registry within 15 days of hiring a new employee. The report must include the company name, address, and Federal Employer Identification Number (FEIN), along with the employee’s name, address, Social Security number, date of birth, medical benefits availability and date of availability, and first day of work.
Employers with workers subject to income withholding or garnishment orders must also provide the state with an address where those orders should be sent, if that address differs from the company address already entered in QuickBooks.

Federal Identification Number
Iowa requires the FEIN to include a 3-digit suffix as shown on the employer’s Iowa label or tax return. The expected format is NN-NNNNNNN-NNN. QuickBooks does not automatically append this suffix, so users must enter it manually on the form.
Employee Date of Birth
QuickBooks will pull the date of birth from the employee setup record, but only if it was entered there beforehand. This is a required field on the Iowa form, so employers who skipped that field during employee onboarding will need to complete it before the report can be filed.
Employee Date of Hire
The date of hire is also imported from the employee setup. However, the form allows users to edit this date directly if the imported value needs correction.
First Day of Work
Federal law made this a mandatory field starting January 1, 2012. The first day of work is defined as the date the employee first performs paid work. Because most employers treat the hire date and the first day of work as the same, QuickBooks automatically transfers the value from the Date of Hire field into the First Day of Work field. Users can override this if the two dates differ.
Medical Benefits Availability
This is a required field. Employers must enter “Yes” if medical benefits are available to the employee or “No” if they are not.
