Form IL-941 in QuickBooks: Illinois Quarterly Withholding Return Explained
QuickBooks prefills most of Form IL-941 for quarterly Illinois withholding filers, but sequence numbers, W-2G, and 1099 amounts may need manual entry.

QuickBooks users who process payroll in Illinois need to file Form IL-941 — the Illinois Withholding Income Tax Return for Quarterly Filers — to report state income tax withheld from employee wages each quarter. The form is generated through QuickBooks’ payroll tax preparation workflow, and while the software handles most of the data entry automatically, several fields require manual review before filing.
Who Must File
Employers who paid wages or salaries subject to Illinois withholding and have been assigned a semi-weekly or monthly due date schedule must file Form IL-941 each quarter. The form reports total state income tax withheld during the quarter to the Illinois Department of Revenue.
Filing is required every quarter — even if no tax was withheld during a given period. A zero-return obligation still applies.
Filing Deadlines
Form IL-941 is due on or before April 30, July 31, October 31, and January 31 of the following year. When a due date lands on a Saturday, Sunday, or legal holiday, the deadline shifts to the next banking day.
Threshold for Semi-Weekly Filing
If withholding exceeds $12,000 during any quarter, employers must transition to a semi-weekly payment and quarterly return schedule. This change takes effect the following quarter, continues through the remainder of the year, and carries into the next calendar year. Electronic payments become mandatory at that point.
What QuickBooks Prefills
QuickBooks automatically populates most fields on Form IL-941 using existing company, payroll, and employee data. In most cases where records are current and complete, no additional manual entry is needed. The Federal Employer Identification Number, for example, is prefilled directly from company setup and must match the employer’s assigned Federal EIN.
Fields Requiring Manual Review
Several fields fall outside what QuickBooks can determine automatically and warrant close attention before submission.
Sequence Number
QuickBooks defaults the Sequence Number field to “000.” In certain situations the correct value may be “001” or “777.” Users should verify the appropriate number for their filing situation and adjust it if the default does not apply.
Total Number of W-2 Forms
This field is relevant only for fourth-quarter or final returns. Enter the total count of W-2 forms reporting Illinois withholding that were required to be sent for the entire year. QuickBooks does not calculate this figure automatically for the annual total.
Wages Subject to Withholding
QuickBooks prefills the amount of wages subject to withholding from payroll data. However, if the employer also issued W-2G or 1099 forms with Illinois withholding, those amounts must be added manually to the prefilled figure on Line 1. Failing to include these supplemental amounts will result in an underreported total.
Business Stopped Withholding
If the business has permanently stopped withholding or paying wages, a checkbox on the form must be marked and the cessation date entered. This applies to employers who have ceased payroll operations entirely. The date entered should reflect when wage payments subject to withholding stopped.
Common Pitfalls
The most frequent issues users encounter center on the supplemental withholding amounts from W-2G and 1099 forms. QuickBooks pulls wage data from standard employee payroll records but does not automatically aggregate gambling withholding or contractor withholding that may also apply to the Illinois return. Users who issue these forms need to manually reconcile the totals.
Another common oversight involves the annual W-2 count on fourth-quarter returns. Since QuickBooks does not compile this number for the full year, employers must track and enter it independently.
